Here are a few tips to remember while completing your application.  A completed and signed application is necessary for an accurate determination of eligibility.  Please provide an answer to all questions and do not use any correction fluid on any forms.  If you do make a mistake simply line through the mistake, make the correction and initial.

A nonrefundable application fee of $35.00 for all adult household members must be submitted with your application.  Please submit the application fee in the form of a cashier's check or money order.  We do not accept cash or personal checks.  We will be unable to process your application without this fee.
