At LaSalle Healthcare Center, we strive to emphasize "home" as much as

We pride ourselves on providing quality, compassionate care in a secure
setting. As part of a national network of long-term care facilities, we
have the expertise and resources to provide a level of care appropriate to the
needs of our residents, from personal care with minimal daily living assistance
to the most skilled nursing treatment for the medically complex patient.
Our services are designed to help residents who require rehabilitative services,
long-term skilled care or treatment for Alzheimer's disease and related
dementia.
Our goal is to help our residents improve their functional ability and
independence. Our staff includes medical specialists, nurses,
nutritionists, social workers and dietitians who work with your physician to
establish a comprehensive treatment plan.
Kindred's on-site clinical pharmacists oversee each patient's drug regimen
and monitor changing pharmacological needs. A full range of pharmacy and
infusion services is available as needed. We also want our residents to
feel comfortable in their home. We offer activities designed to stimulate
the whole person. We have a dining room where residents can enjoy a meal
or a cup of coffee. Family members are always welcome to come and share a
meal or visit.

From the Durham Freeway (Highway
147), take exit 15B, the Duke University Medical Center and VA Medical Center
exit. Take a left (South) on Fulton Road until it ends at Erwin Road in
front of the Duke Medical Center. Take a right onto Erwin Road, and take
another right on LaSalle Street. LaSalle Healthcare Center is located at
411 South LaSalle Street.
